Preface
The etiology of periodontal diseases is well understood, and we have now developed efficient methods for prevention, treatment, arrest, and control of these diseases. For example, over the last 27 years in County of Värmland, Sweden, large-scale implementation of our preventive programs has led to an increased number of remaining teeth in 65-year-old patients by more than 15% and a reduced loss of periodontal support by more than 20%, at the same time reducing the percentage who are edentulous from 17% to 7%. In addition, in a 15-year longitudinal adult study, we maintained the periodontal attachment level unaltered irrespective of age. 

According to the principles of lege artis, all members of our profession are obliged to offer treatment based on the most current scientific and clinical knowledge available. As we now have entered the new millenium, we must therefore continue to concentrate our efforts on preventing, controlling, and arresting the development of dental caries and periodontal diseases. However, needs-related preventive and maintenance programs must be cost effective and should be based on information derived from comprehensive diagnoses, histories, and risk predictions and group, individual, and tooth-surface levels. For quality control and evaluation of such programs, computer-aided analytical epidemiology, using relevant variables, should be introduced.

The aim of this book, the third volume of a five-volume series of textbooks and atlases, is to give the reader updated knowledge about the etiology, modifying factors, risk evaluation, pathogenesis, diagnosis, and epidemiology of periodontal diseases, as well as the relationship between periodontal diseases and other diseases. 

Sufficient knowledge about the above topics is the first prerequisite for efficient and need-related prevention, treatment, arrest, and control of periodontal diseases. New materials and methods for prevention, nonaggressive treatment, arrest, and control of periodontal diseases are comprehensively discussed and illustrated in volumes 4 and 5. 

In this book, detailed scientific backgrounds, well-illustrated guides to implementing the "state of art,” and conclusions and future recommendations are provided for each topic addressed. Thus, the book is useful not only for dentists and dental hygienists, but also for undergraduate and postgraduate students. 

In the chapter on etiology of periodontal diseases, the updated evidence-based role of nonspecific plaque formation rate, the specific periopathogenic microflora, and ecologic conditions, as well as new microbiologic tests such as the "checkerboard” DNA-DNA hybridization technology, are discussed. 

New developments in the relationship between external (environmental) and internal (endogenous) risk indicators, risk factors, and prognostic risk factors, as well as principles for risk prediction and establishment of risk profiles as a tool for communication with the patient, are comprehensively discussed and illustrated. In particular, some genetic factors, in combination with smoking, are discussed in detail as the most important modifying risk factors. 

New research on periodontal diseases as a possible risk indicator, risk factor, or prognostic risk factor for coronary heart diseases, stroke, infective endocarditits, pneumonia, preterm low birth weight, and poorly controlled diabetes is discussed. The new system for classification of periodontal diseases, which is used throughout this volume, is presented, and recent research on the pathogenesis of periodontal diseases is described step by step and well-illustrated from clinical and cellular levels down to the molecular level. 

In the chapter on diagnosis of periodontal diseases, techniques and validity of clinical probing of attachment loss versus radiographs are discussed and illustrated. New tools for computerized digital subtraction radiography, controlled-force probing, measurement of periodontal pocket temperature (fever) and gingivae fluid amount, and content of molecules related to active periodontal destruction are also discussed and illustrated. 

Finally, the great importance of analytic epidemiology at population, group, individual, and tooth-surface levels for quality control, outcome of therapy, design, and evaluation of prevention and maintenance are discussed. 

This project could not have been completed without the assistance and support of my family, friends, and colleagues.
